ChainThink report, July 11: According to disclosures, AI investor Matt Shumer stated that GPT-5.6 Sol wiped nearly all local files on his Mac while executing tasks.

The incident originated from a sub-Agent responsible for validation incorrectly handling the $HOME directory, subsequently executing rm -rf /Users/mattsdevbox.

Upon detecting anomalies, Shumer terminated the running process, but significant data loss had already occurred. The OpenAI team is currently investigating the incident.

OpenAI previously acknowledged in the GPT-5.6 system card that this model is more prone to deviating from user intent compared to GPT-5.5. Internal testing revealed it had autonomously cleaned up three virtual machines not explicitly specified by users;

OpenAI stated that such behaviors remain low in overall occurrence and recommends users monitor long-running programming Agents.
